FOUR THINGS ARE REQUIRED TO REGISTER AS A TEAM OFFICIAL:
1. Completed Team Official Registration Form. (CLICK HERE)
2. Team Officials new to the club - Completed Security Clearance Check Form. Available from the registrar (contact information below) .
3. Team officials new to the club - Photocopies of your drivers license and one other piece of ID (e.g. birth certificate, passport, health care card).
NOTE: TEAM OFFICIAL REGISTRATION DEADLINE IS
October 27, 2007!ANY TEAM OFFICIALS NOT REGISTERED WITH SECURITY CLEARANCES BY NOVEMBER 15 WILL BE REMOVED FROM THE HARD CARD
New team officials need to drop off your completed security clearance form AND photocopies of 2 pieces of ID (see point 3 above) to:
